Prime Quick Login on CA_Black Style

I have a style called CA_BLACK, can someone help me install Prime Quick Login? I just cannot get any of my mods to work on this theme, and we've had it forever and so I finally switched to hosting where i can edit and install my own stuff... but I cant get anything to work.

Please help!

Hello and welcome, Saved0ne. I went ahead and gave this it's own thread.

What happens when you install it? Do you get an error or anything? Also, did you clear your cache after installing it?
