Custom field for post

Hello, I need a way to have my users create new thread topics that have custom content that they fill out with a field, like drop down menus, and checkboxes, similar to a ticket system. Something similar is done here using IP Board, http://cdsdraft.com/mocks/member-mocks/index.php?id=8511 to create your own you would have to create a member, you can use mine, kookabura3, matthew2414, I know its possible because they are doing it, so if someone could help me out I would really appreciate it.
