confirmation codes shows as ?

Has anyone else had this problem? Any ideas what causes it??

I managed to fix this too by changing to a q&a type confirmation but no idea what caused the original issue. users on phpbb told me it was out of date and to install captcha but I didnt fancy the challenge!!
